I, MARCELINA B. ESCALANTE, of legal age, married,

Filipino, and a resident of Pagadian City, Zamboanga del Sur, after
having been duly sworn to in accordance with law, do hereby depose
and say:

1. That I am the mother of Gerlin Escante Getigan;

2. That I am the beneficiary or dependent of Gerlin Escalante

Getigan in her PhilHealth coverage;

3. That I am currently confined at Metro Pagadian Hospital

and I would wish to avail of the PHilhealth coverage under
the name of my daughter. It appears however, that my name
as appearing in my Certificate of Live Birth is Marcelina
Babao Escalante while my name in the Certificate of Live
Birth of Gerlin Escalante Getigan is Marceliana Babao
Escalante;

4. That I hereby attest that the person named as Marcelina and

Marceliana actually refer to one and the same person and
that my real and correct name is Marcelina Babao
Escalante;

5. That I am executing this affidavit to attest to the truthfulness

of the foregoing statements and for any legal purpose(s) this
may serve best.

WE, LUCRECIA GULANE ESTUDILLO and PATRICIA

OGDOL TAN, Filipinos, both of legal age, married and residents of
Purok Cacao, Tuburan, Zamboanga del Sur, after being duly sworn to
in accordance with law, hereby depose and say:

1. That we personally know MARCELINA BABAO

ESCALANTE, she being our neighbor and acquaintance for a
long period of time;

2. That, by inadvertence, her name was erroneously entered in as

MARCELIANA BABAO ESCALANTE instead of
MARCELINA BABAO ESCALANTE, her true and correct
name;

3. That we are certain that the names MARCELIANA BABAO

ESCALANTE and MARCELINA BABAO ESCALANTE
refer to one and the same person that is MARCELINA
BABAO ESCALANTE;

4. That we are not related to MARCELINA BABAO

ESCALANTE, either by consanguinity or affinity, nor do we
have any pecuniary interest in this matter whatsoever;

5. That finally, we are executing this affidavit to attest to the

truthfulness of the foregoing statements and for any legal
purpose this may serve best.
