Dear Sir,

I made this letter of explanation to clarify the reason why I was not able to time/log out in our school’s
biometric last April 2, 2017. On that day, I logged in at 8:59 AM and been busy for the practices for
moving up of the grade 10 the whole morning. It’s just that after our practices, I went to the City Hall to
follow up our diploma jacket with the school vehicle. When we were done, I decided to leave it to the
driver to send those things in our school alone and traveled going home since the terminal is near in our
location and it was already 5 PM. It wasn’t my intention to forget to logged out. It just that I thought, I
already did.
I sincerely apologized for what I did sir. Rest assured that it will not happen again.

I am hoping for your kind consideration.

God bless.

Sincerely Yours,

NYMPHA B. DECORINA
