I, ROSE MAE FLOR P. TAEZA, 30 years old, married, Filipino and a

resident of Purok Rose, Brgy, Baras, Tacurong City, Sultan Kudarat, after
having sworn to in accordance with law, do hereby depose and state:

1. That, I was appointed as Barangay Secretary of Barangay Baras,

Tacurong City last 2013 and took an oath on the said public office on
the same year;

2. That, during my term of service, I had never been employed in the

government or service requirement and did not received any form of
salary or compensation, except honorarium being a Barangay
Secretary of the above-mentioned barangay.

3. That, I am executing this affidavit to attest the truthfulness and

veracity of the foregoing statements and for the purpose of applying
the eligibility grant for public officials before the Civil Service
Commission.

I, ROSE MAE FLOR P. TAEZA, 30 years old, married, Filipino and a

resident of Purok Rose, Brgy, Baras, Tacurong City, Sultan Kudarat, after
having sworn to in accordance with law, do hereby depose and state:

1. That, I was appointed as Barangay Secretary of Barangay Baras,

Tacurong City last 2013 and took an oath on the said public office on
the same year;

2. That, I, being appointed Barangay Secretary of the above-mentioned

barangay is not related up to the 4th degree of consanguinity, or of
affinity, to the appointing Punong Barangay;

3. That I am executing this affidavit to attest the truthfulness and

veracity of the foregoing statements and for the purpose of applying
the eligibility grant for public officials before the Civil Service
Commission.
