AFFIDAVIT OF LOSS
I, RUSSEL D. MATI, of legal age, married, resident of Blk.3, Lot. 1 Baluarte Hills
Subdivision Tamag, Vigan City, Ilocos Sur, Philippines, and the President of ARTHIT Agro
Supply and Machineries Corp. with office address at Taleb, Bantay, Ilocos Sur, after having been
duly sworn to in accordance with law do hereby declare and state:

1. That I have been issued an official receipt from the Provincial Treasurer’s
Office of Ilocos Sur with number 9686969 dated December 28, 2016 in the
amount of Seven Million Four Hundred Eighty Three Thousand Five Hundred
Pesos (Php 7,483,500.00) as payment for the performance bond;

2. That I kept the said receipt in an envelope and put inside my filing cabinet
together with the other files in the office;

3. That when I needed the said receipt, I can no longer locate it in the filing cabinet
despite diligence and utmost effort;

4. That I even asked for the help of my employees to locate the same but all our
efforts proved futile and in vain making me conclude that the said official
receipt has been lost and is beyond recovery;

5. That I am executing this affidavit to attest to the truthfulness of all the foregoing
and for the purpose of seeking replacement of the said for all legal intents it
may deem serve.

IN WITNESS WHEREOF, I have hereunto set my hand for signature this 10th day of
October, 2017 at Vigan City, Ilocos Sur.
