A. PROJECT CLOSE-OUT

During the period of the Contract, Energy Equity Epic Sengkang Pty.Ltd.(EEES)
will issue Contract Award to PT Borneo Mitra Sinergi (BMS) to covering all required

matters for the performance of each specific Work or part of the Work by BMS including
detail of scope of Work, items to be provided by BMS, commencement date, milestones,
completion date, maximum or lump-sum compensation, penalty, etc. Both EEES and BMS
will agree and sign Document Contract before the commencement of the Services.

A.1. Close Out

Project Work should be done and accomplished and after that should be reported. After
finished, all document should be signed by BMS representative persons and submitted to
EEES for approval.

A.2. Document Record

During the performance of project work BMS will issue the timesheet for labour and
equipment by daily until the work is finish. Those timesheet should be approved by EEES
representative persons and BMS will record those approved timesheet for Total man hour
and equipment km driven.

A.3. Material Reconciliation

BMS will keep records of materials supplied by EEES and issued to perform of the
service. At the time of completion of Services, BMS will be required to reconcile materials
supplied by EEES with materials incorporated into the Services, materials returned to
EEES and including scrap.

A.4. Completion of Services

A.4.1. Mechanical Completion

Mechanical Completion means that the Services for such Facility is:
Complete – the Facility have been installed in accordance with specifications, design
drawings, and other written requirements of EEES.

Tagged – all signs, stencils, and tags required for identification are complete

Tight – there is no leakage from equipment, piping, flanged joints, etc.

Adequately tested – both in shop and in field

Safe – all systems for the protection of personnel and property are installed.

Clean – the Facility has been cleaned.

Checked and in working order – including loop check, instrument calibration, electrical
control check, lubricants added to machinery, equipment alignment and operability check,
etc.

Ready to be operated or used – the Facility has been inspected and is ready to be operated
or used in accordance with specifications, design drawings, and other written requirements
of EEES.

Well documented –documents has been handed over to EEES by BMS and accepted by
EEES.

Documents which shall be handed over to EEES as the provision of mechanical

completion will consist of original document and three (3) copied documents and
electronic file as follows:

 As built condition drawings.

 Documentation of field change notice/report

 Documentation of scope change notice/report

 Inspection report

 Precommissioning and commissioning report

 Purchase order

 Manuals, data sheets and drawings of equipment if the equipment

purchased.

 Material certification if the material purchased.

 Material heat tracing report

 Material Safety Data Sheet (MSDS) if the material purchased.

 Equipment Certification (MIGAS, Depnaker, etc) if such

certification is BMS scope or its Subcontractors Documentation of
non-conformance report, if any.

A.4.2. EEES will issue a Certificate of Mechanical Acceptance to BMS after

requirement mentioned above is completed and handed over to EEES.
