AFFIDAVIT OF GUARDIANSHIP

I, CARLA B. YEE, 23 years of age, single, Filipino and a resident of R.V. Fulache
Street, Hilongos, Leyte Philippines after having been duly sworn to in accordance with
law, hereby depose and state that:

1. I am the sister of CLARINA B. YEE, 17 years of age, Filipino and who have
interest in applying for the DOST –SEI Science and Technology Undergraduate
Scholarships;

2. That the above applicant is my sister and is under my care and custody;

3. That I am competent to certify in behalf of our parents in her application for

the above-mentioned DOST Scholarship;

4. That I am not imbecile, insane, a vagrant or a vicious person or a habitual

drunkard or a habitual criminal, and have not abandon, neglected or refused to
support said applicant or cause her to commit offenses against the law;

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and to support the application of my sister for the said DOST
Scholarship and for whatever legal purpose it may serve.

IN WITNESS HEREOF, I have hereunto set my hand this 6th day of September
