MEMORANDUM OF AGREEMENT

KNOW ALL PERSONS BY THESE PRESENTS:

This Memorandum of Agreement made and entered into this 3th day of October 2017 at San
Carlos Heights, Upper Irisan, Baguio City by and between:

The Department of Education-San Carlos Heights Elementary School represented by its

school head, Florida F. Cael and School-Based Feeding Program Coordinator, Joyce P. Lawaguey
herein referred to as SCHES;

And

The School Canteen represented by Mr. Mario E. Andayan, the canteen manager herein
referred to as School Canteen:

WITNESSETH

WHEREAS, DepEd SCHES, a government agency and existing under the laws of the Republic of the
Philippines is located at Purok 20 Upper Irisan, Baguio City.

WHEREAS, the school canteen located at the school grounds of SCHES refers to one of the ancillary
services in the school system that sells food items to the pupils and serves as support mechanism to
the Department’s response to the mandate of Article 2 of Republic Act No. 6938;

WHEREAS, the school canteen shall help eliminate malnutrition among pupils as stipulated in DepEd
Order No. 8 s. 2007;

WHEREAS, the Department of Education (DepEd) through the representation of SCHES, in its
mandate to address undernutrition among public school children implements the School-Based
Feeding Program (SBFP) for school years 2017-2022;

WHEREAS, the budget allocation for feeding is Php18.00 per beneficiary multiplied by the number of
feeding days (120);

WHEREAS, the parties seek to help and cooperate to undertake the implementation of the Program;

NOW THEREFORE, premises considered, the parties hereby agree on the following terms and
condition of the Memorandum of Agreement:
1. The Program

This Agreement outlines the details of the Program to be implemented between the School
Canteen and SCHES.

A feeding that comprises of lunch for thirteen (13) beneficiaries good for 120 days from
kindergarten to grade VI.

2. Duration
The Program will start by October 6 2017 till March 30, 2018 only.

3. Budget
The amount allocated per meal is Php 18.00 per beneficiary.

4. Procedure
Lunch will be served every 12:00nn, utensils needed will be prepared and provided and so
with the aftercare by the School Canteen.

IN WITNESS WHEREOF, the parties hereto have executed this Agreement Effective as of the date
above written.
