Affiliate Program Questionnaire

The purpose of this questionnaire is to gather all the necessary information required to set up a
strategy to increase your company’s sales through leveraging the power of affiliate recruitment.
This questionnaire has been designed to get a good understanding of your products and current
business so that we can set up the strategy correctly and quickly. It’s fine if you do not have the
answer to every question, but please fill it out as much as you possibly can. We may have
already discussed some items on the phone but having it in writing will be helpful for our
records.

1. Please send the links to all your web properties (websites, sales funnels, social media
accounts).

2. Which digital advertising channels are you currently utilizing? How are each of them
performing? Please provide a link to the specific funnels (sales pages and flow of sales)
set up for each specific channel.

3. Which products do you want to sell through the affiliates programs? Please also add
links to each. Rank them by popularity/best-selling if possible.

7. What are the conversion rates on the products sales pages?

9. How much can you currently afford to acquire a customer?

10. What keeps your customers up at night?

11. Why are your customers purchasing from you instead of a competitor?

12. Do you have an email marketing campaigns set up to generate back end sales?
13. What is your budget to invest in setting up an affiliate program?

14. How many sales a day can you handle on the products you want to sell?

15. What is your fulfillment process? Can you fulfill large quantities of each product quickly?

16. What are your monthly revenue objectives for the affiliate programs by months 3/6/12?

17. Are there any places you do not wish affiliates to advertise your products on?

18. Do you have any marketing materials you can provide affiliates (pages, banners, etc…)?

19. Any other data or information you would like to share that you believe is important for
us to know?
