In consideration thereof, I promise to pay the approved loan/s in accordance with this Loan Application, Agreement and
Promissory Note and hereby bind myself to the following TERMS & CONDITIONS:

1. ELIGIBILITY: Regular employee of the Bank who at the time of filing of this loan application:
 Rendered a continuous service of (a) 6 months for CL/MFL/HMOF/RAF (b) 3 years for
APL & GEL; (c) 5 years for LRF & SBL
 MNTHP of at least ₱ 5,000 after deduction of loan amortization
 NOT on continuous leave without pay (LWOP) for at least six months
 NOT on absence without official leave (AWOL)
 NO pending administrative charge with maximum imposable penalty of dismissal or
under preliminary investigation by the Administrative Legal Department and there
exists a prima facie case for an offense punishable by dismissal
 NO past due account with LBP Provident Fund
 NO outstanding Loan Take-out balance, in case of LRF application
 NOT disqualified pursuant to existing PF rules and regulations
2. INTEREST RATE & Interest rate per annum on loans and the maximum term shall be as follows based on

3. MODE OF Payment of the loan shall be in equal monthly amortization and automatic through salary
PAYMENT: deduction, the first of which shall be due on the month immediately following the month of
loan release. As such, the applicant-borrower hereby assigns and authorizes the Provident
Fund Department, Administrative Accounting Department, Personnel Administration
Department, or any authorized unit of the Land Bank of the Philippines TO DEDUCT the
corresponding MONTHLY AMORTIZATION of the loan from applicant-borrower’s salary
every payroll period until the loan is fully paid.
4. COLLATERAL The loan shall also be secured by the Real Estate Mortgage previously executed, if any, or to
be executed, to secure the Housing Loan, Special Financing Program Loan and/or Programs
for Shelter Enhancement of the member-borrower (For CL, GEL and LRF)

5. ESCALATION OF Interest may be increased at any time in the future by the LBP Provident Fund without prior
LOAN INTEREST: notice in accordance with the prevailing rules and regulations and that the applicant-
borrower hereby consents and agrees thereto.
Note: In case of HMOF application, Official Receipt (OR) issued by the HMO Provider must
be submitted to PFD WITHIN ONE MONTH from the release of the loan proceeds.
Non-submission of the OR shall be subject to interest rate escalation of 4%.

6. SURCHARGE: Any unpaid loan amortization shall bear an additional interest rate of 2% per month from
the date it became due and demandable until full payment.
7. INSURANCE The amount of loan granted shall be covered by the LBP Provident Fund’s self-insurance and
GUARANTY FUND: guarantee schemes and the amount of premium shall be deducted from the loan proceeds.
8. SECESSION OF Regardless of the manner of separation (resignation, Separation Incentive Program,
MEMBERSHIP: dismissal for cause, death, etc.), this loan shall become due and demandable.
9. ATTORNEY’S FEES: In the event this obligation is not paid in accordance with the terms and conditions of the
agreement and the LBP Provident Fund is obliged to institute a court action to enforce its
collection, the applicant-borrower agrees to pay as attorney’s fee an amount equivalent to
10% of the total amount due including interest at the time of the institution of the action
(inclusive of cost and expenses allowed by law).
10. In the event a suit in the enforcement of collection in accordance with the terms and conditions of this
AGREEMENT should arise, court action shall be instituted exclusively in the appropriate court in the City of
Manila.
11. LBP PROVIDENT It is understood that the loan is subject to the existing PF Rules, Regulations and Policies as
FUND RULES & well as those which will be adopted by the LBP Provident Fund in the future.
