AFFIDAVIT OF LOSS

I, ALEXANDRA NICOLE D. SUGAY, of legal age, Filipino, resident of Manila, single, under oath state:

1. I am the owner of a UST Student ID with control no. 2014 – 063334.

2. On December 14, 2017, my ID was attached to my lanyard. On that day, I went to Miguel de Benavides
Library. I tapped my ID upon entering the premises.

3. I stayed at the Civil Law section on the third floor. I went there to borrow Taxation books for my final
examination.

4. I sat at the right most part of the room and stayed there for three hours.

5. At around 5 pm, I left the library and attended my class in St. Raymund’s Building.

6. I did not tap my ID upon entering because the guard was not looking.

7. It was only around 7 pm when I realized my ID was unattached to my lanyard.

8. Despite diligent search, I am unable to find my ID.

9. I am executing this affidavit to support my application for a new ID.

Re: Final demand to return Ms. Ramacola’s G-tec

On behalf of my client Ms. Marielle Ramacola, I write regarding the G-tec she lent to you.

Based on a Lease Agreement dated 01 January 2017 that you entered with my client, you were able to
borrow her ballpen with point 0.3. According to the agreement, you were supposed to return it on 27
December 2018.

I gathered from my client that you have not returned the ballpen despite written and verbal demand.
Thus, demand is hereby made upon you to return my client’s ballpen within five days from receipt of this
letter. Should you fail to return to return the ballpen, I will not hesitate to take necessary action to protect
my client’s interest.
