AFFIDAVIT OF WAIVER

I, STEFANIE CRISTY BUIT, filipino, of legal age, single, resident of Dapa,

Surigao del Norte after having sworn to in accordance with the law, hereby
depose and state, THAT -

I am a graduate of Bachelor of Secondary Education and a Licensed

Teacher.

That I apply at Dapa National High School as a volunteer teacher for the
school year 2017-2018.

That being a volunteer teacher I cannot received any salary but rather a
good experience of teaching and a camaraderie with students and co-
employees of the institution.

That as a volunteer teacher, I will abide in all the rules and regulation
implemented by the school to all teachers and employees.

That in case of injury incurred to me while inside the school campus, the
school is not liable thereof.

That this waiver of rights is made with my knowledge and further understand
that I may waive whatever right I have in this agreement.
