AFFIDAVIT OF COMPLAINT

I, MIRAFE DOCTOLERO MANZON, 44 years old, married, a Pharmacist and

residing at Door A1, Conchita Ville, Barangay Nangka, Consolacion, Cebu, Cebu, after
having been duly sworn to in accordance with the law, do hereby say;

1. That I’m here to press charge against my husband EDUARDO SARAGENA

MANZON, 46 years old, married, a seafarer, a native of Langtad, Argao,
Cebu and a residing at Door A1, Conchita Ville, Barangay Nangka,
Consolacion, Cebu., and presently employed at SYNERGYGROUP
OPERATIONS INC., Located at 3rd Floor Universal LMS Bldg, #106
Esteban Street, Legazpi Village, Makati 1229, for VIOLATION OF R.A.
9262.

2. That on July 18, 2009, we got married and solemnized by Rev. Diosdado R.
Manzano of Proclaiming the Good News Ministry Inc., and it was witness by
our respective families and friends and blessed with three (3) children
namely: Eden Mae Doctolera Manzon, 11 years old, Eduana Marie Doctolera
Manzon, 8 years old and Edrah Marielle Doctolera Manzon, 4 years old,
presently under my custody. (Attached herein NSO Marriage Contract and
Certificate of Live Birth of children).

3. That June 15, 2017 around 6:30 in the morning, I was in the house preparing
things before going to work, my husband Eduardo and my nanny Editha
Remulta had a heated arguments and it comes to the point that I pacified
them but my husband(Eduardo)got mad at me. Then I together with my three
(3) children and nanny Editha, I delivered my children to their respective
school and my nanny Editha left in the school of my youngest daughter Edrah
Marielle.

4. That on the same date around 11:00 in the morning, when my nanny Editha
and daughter Edrah Marielle, leave the school, I give instructions to her that
they will not go inside the house and stayed only at our neighbors house if my
husband was still inside, so my nanny Editha peep at the door and she saw
my husband Eduardo packing his things and when she (Editha)return to
check around 2:00 in the afternoon, she found out that my husband Eduard
is no longer inside the house. That after work around 6:00 in the evening,
when I arrived home, I immediately check our room and I found out that all his
personal belongings was no longer inside our cabinets.

5. That I excited all my efforts to contact my husband Eduardo of his

whereabouts but still I failed and since June 20, 2017, my husband Eduardo
didn’t show to us his family nor give us financial support. That on October 8,
2017, I was informed by our common friends that my husband Eduardo was
already working aboard and left the country on the 3 rd week of August 2017.
After knowing the information given, I was worried, emotional abuse, of what
he did to me and my children and for not showing to us or contact and only to
find out he was already working aboard.

6. That on October 12, 2017, I decide to report at Consolacion Police and

complaint against my husband Eduardo for financial support and I was given
advice to refer my case to Philippine Overseas Employment Administration
(POEA) Regional Center for Visayas for get some documents and base on
record my husband Eduardo was processed by the agency mentioned above,
sometime August 25, 2017 to work as OILER for the foreign principal
SYNERGY MARINES PTE LTD with a nine (9) month/s contact. (Attached
herein OFW Record Information).
 7. That the office of POEA, Regional Center for Visayas, made a demand letter
dated November 3, 2017 address to the agency of my husband Eduardo but
no reply from the agency. (Attached herein Letter to Demand for Legal
Support).

8. That on April 30, 2018, I was given referral by the Consolacion Police to
undergo Psychological evaluation at Don Vicente Sotto, Memorial Medical
Center, Cebu City. (Attached herein the letter request.)

9. That I have nothing more add nor retract from the given statement and I am
executing this affidavit voluntarily to support my complaint against EDUARDO
SARAGENA MANZON, for VIOLATION OF R.A. 9262, in the court of law.

of Door A1-Conchita Ville, Brgy Nangka, Consolacion, Cebu, personally appeared to this
office and alleged that his husband Eduardo Saragena Manzon, 46 y.o, married, a
seafarer and a native of Langtad, Argao, Cebu, left their house last June 15, 2017
around 2:00 PM, because they had conflict with financial matter and jealousy of the
reportee’s husband from that time his husband (Eduardo) never showed nor contact his
family. Further alleged by the reportee that last October 08, 2018, reportee learned that
his husband work abroad and left the country on the 3rd week of August 2017. Reportee
wanted financial support for his husband for their three(3) children namely: Eden Mae
D. Manzon, 11 y.o, Eduana Marie D. Manzon, 7 y.o and Edrah Marille D. Manzon, 4 y.o.

The above-named accused, EDUARDO SARAGENA MANZON, 46 years old,

married, a seafarer, a native of Langtad, Argao, Cebu and a residing at Door A1,
