provides health insurance for the elderly population
as well as those with certain disabilities

AGE RANGE & PREREQUISITES FOR SERVICES

Medicare is a health insurance program for:

★ People age 65 or older


★ People under age 65 with certain disabilities
○ ALS (Lou Gehrig's disease)
★ People of all ages with end-stage renal disease
(permanent kidney failure requiring dialysis or a
kidney transplant)
MEDICARE PART A

Hospital services covered:


★ Inpatient care in a hospital
★ Skilled nursing facility care
★ Inpatient care in a SNF
★ Hospice care
★ Home health care
MEDICARE PART B
Part B covers 2 types of services:
★ Medically necessary services: Services or supplies that are needed
to diagnose or treat a medical condition
★ Preventive services: Health care to prevent illness (like the flu) or
detect it at an early stage

Services covered:
★ Clinical Research
★ Medicare “Wellness” visits every 12 months
★ Lab test
★ X-ray
★ Orthotics and prosthetic
★ Ambulance services
★ Durable medical equipment (DME)
★ Mental health
★ Inpatient
★ Outpatient
★ Partial hospitalization
★ Limited outpatient prescription drugs
★ Getting a second opinion before surgery
WHAT IS NOT COVERED BY PART A & PART B?
★ Long-term care
★ Most dental care
★ Eye exams related to prescribing glasses
★ Dentures
★ Cosmetic surgery
★ Acupuncture
★ Hearing aids and fitting examinations
★ Routine foot care
MEDICARE PART C
★ AKA Medicare Advantage
★ Administered by private insurance
companies contracted with Medicare
★ Covers the same benefits as Medicare Part A
and Medicare Part B

★ Extra benefits:
○ Fitness benefits including exercise classes
○ Routine dental care (cleaning, x-rays,
dentures)
○ Routine hearing care (hearing aids)
○ Routine vision care (contacts, glasses)
MEDICARE PART D
★ Prescription drug coverage
★ Optional
○ Late penalty
★ Must be enrolled in Medicare A or B

★ Medications covered:
○ All commercially available vaccine drugs to prevent illness
○ Depends
★ Drugs not covered:
○ Weight loss or weight gain
○ Cosmetics & hair growth
○ Fertility drugs
○ Sex or erectile dysfunction
○ Over the counter
UTAH LOCATIONS
★ Division of Aging and Adult Services – This division focuses on
services and programs specifically for senior residents in the state.

★ Utah Department of Health – This department provides assistance


for beneficiaries who have difficulty paying their Medicare
premiums and deductibles

★ Utah Aging and Disability Resource Connection – This office serves


as a central place to get information that Utah Medicare
beneficiaries may need. The network also provides resources for
disabled residents of the state

INFO REQUIRED FOR MEDICARE SERVICES
★ Your birth certificate
★ Proof of United States citizenship or legal residency
○ You must be either a United States citizen or a legal permanent
resident of at least five continuous years
★ Social Security card if you are already receiving benefits
★ Health insurance information about the type and dates of coverage
★ Information about your employment, such as a W-2 form, if you are still
working
★ US military discharge papers if you served before 1968

If you sign up for Medicare online or by phone, you may need to submit additional
information. Social Security will let you know if you need to do this and what that
information might be
