AFFIDAVIT OF CONSENT
I, TOMI GALE BANZUELO CRUZ, of legal age, Filipino, Married to
HARPREET SOHAL, with residential address at Luna Street Tandang Sora Apt.,
Zone, Digos City, Davao del Sur, Philippines, after having been sworn to in
accordance with law, do hereby depose and state that:

1. I am the mother of the minor, THOM HARVIS B. CRUZ, who was born
on the 8th day of December 2007 and bona fide student of Philippine
Nikkie Jin kai International School;

2. I am giving my consent for my son to travel abroad to be accompanied by

my sister, KHEN BEVERLY CRUZ NIELO, who is of legal age, Filipino,
Married to ANDRES NIELO JR. and also a resident Blk 5, Lot 2, Davao
Empress Subdivision, Sasa, Davao City, Philippines;

3. As such, my son is invited by my sister and her husband to travel with

them out of the country in the following destinations to wit, Japan,
Thailand, Singapore, Hong-kong and Taiwan within the year of 2019;

4. During the said travel, My son is under the control and custody of my
sister KHEN BEVERLY CRUZ NIELO and his husband ANDRES NIELO
JR.;

5. I guarantee that she will not violate any existing laws of the countries
above-mentioned;

6. I hereby declare the truth of the foregoing parental consent for my son
THOM HARVIS B. CRUZ to travel with them to the above mentioned
countries and to the above-mentioned date;

7. I am executing this affidavit to attest and declare the truth of the

foregoing facts and for whatever legal purposes that this affidavit may
best serve.
