JOINT AFFIDAVIT
(Re: Late/Delayed Registration of Birth)

WE, EUGENIO G. HAYAG and JESUSA R. GERUNDIO, both of legal age, Filipinos,
and residents of Ibabao-Estancia, Mandaue City and Nangka, Consolacion respectively, after
having been sworn in accordance with the law hereby depose and state;

1. That we know ANNIKA ZOUE HERANA GOC-ONG who was born on August 20,
2010 in Eversley Childs Sanitarium and General Hospital, Jagobiao, Mandaue City,
Cebu to MERRYJOEY ERMAC HERANA and JAY HERBIETO GOC-ONG;

2. That due to inadvertence and failure to have immediate follow-up on the part of his
parents and the prevailing conditions then, her birth was not reported to and
registered with the Office of the Local Civil Registrar of Mandaue City immediately
thereafter;

3. That we have firsthand knowledge of the foregoing facts because we have been
close friends and neighbors with her parents for a long time;

4. That we are not related to ANNIKA ZOUE HERANA GOC-ONG and her parents
and we do not stand to benefit from the registration of her live birth;

5. That we are executing this Joint Affidavit to attest to the truth of all the foregoing
facts, in connection with the application for the late/delayed registration of birth of the
said ANNIKA ZOUE HERANA GOC-ONG with the Office of the Local Civil Registrar
of Mandaue City, and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 30th day of March 2019
at the Municipality of Consolacion, Philippines.
