Kapiligan Street Barangay Dona Imelda Araneta Quezon City, after having been sworn in
accordance with law hereby depose and state that:

1. I am the Owner of Daniel-Martin’s Pastries;

2. Daniel-Martin’s Pastries is a duly registered sole proprietor’s under Philippine Laws and
was registered with Department of Trade and Industries (DTI) on July 5, 2016;

3. Daniel-Martin’s Pastries has its main store at 154 A. Kapiligan Street Brgy. Dona Imelda
Araneta Quezon City and is primarily engaged in the making of pastries.

4. From Jan 1, 2018 up to present Daniel-Martin’s Pastries has been stopped its operation
due to some personal reason and profitability.

5. This affidavit is executed for the purpose of securing a business retirement certificate
from your good office.
