AFFIDAVIT

I, MICHELLE C. QUIONAL, of legal age, Filipino and a resident of Gomez,

San Isidro, Isabela, after having been sworn in accordance with law, hereby
depose and state that:

1. I am accepting the full consent and authority given by Spouses

Jonathan L. Sadural and Wilma C. Sadural to apply for passport of
their child Mikaella C. Sadural;

2. I likewise accept the consent of the spouses as travelling companion of

their child in Hong Kong;

3. I am executing this Affidavit to attest to the correctness and

truthfulness of the foregoing statements and in compliance with the
legal requirements for travel of minor child.
