AFFIDAVIT OF EXPLANATION AND DECLARATION

(In lieu of Saudi Arabian Poli ce Clearance)
I, KENNY D. KIAT-ONG married, of legal age, Filipino citizen of the Philippines, 38 years
of age, with present address at JC 364 Central Pico, La Trinidad, Benguet, Philippines,
after having been duly sworn to in accordance with law, and hereby states the following;

1. That I am an applicant for overseas employment in New Zealand, through a

POEA accredited agency;

2. That I lived in Saudi Arabia for more than 5 years, and as such one of the
requirements is for myself to submit a police clearance from Saudi Arabia ;

3. That I am unable to obtain a Saudi Arabian police clearance as I am currently

residing in the Philippines and do not have current legal resident status in Saudi
Arabia ;

4. . That I sincerely and truthfully declare that I have never been, or was
never convicted or found guilty of, or charged with any offence against the law of
Saudi Arabia and any other countries;

5. That I am executing this affidavit to attest to the truth of the foregoing

circumstances, for my application to work in the New Zealand and for whatever
legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this March 15, 2019 in the
Municipality of Diffun, Quirino Province, Philippines.
