Mechanism Of Action

>inhibit reabsorption of sodium and water in the ascending limb of the Loop of Henle by
interfering with the chloride binding site of the 1Na+ 1K+ 2Cl-contransport system. Loop
diuretics increases the rate of delivery of tubular fluid and electrolytes to the distal sites
of hydrogen and potassium ion secretion, while plasma volume contraction increases
aldosterone production. The increased delivery and high Aldosterone levels promote
sodium reabsorption at the distal tubules, thus increasing the loss of potassium and
hydrogen ions

Indication/Contraindication
Indications:
>Edema due to cardiac, hepatic and renal disease, burns; mild to moderate HTN,
hypertensive crisis, AHF, reduced urinary output due to gestoses, CRF, nephrotic
syndrome.
Contraindications:
>Anuria; Hepatic coma and precoma; severe hypokalemia/hyponatremia; hypovolemia
w/ or w/o hypotension; hypersensitivity to sulfonamides.
SE/AR
>Low BP >Dehydration and electrolyte depletion >Jaundice >Tinnitus >Photophobia
>Rash >Pancreatitis >Nausea >Diarrhea >Abdominal pain and dizziness >Increased
blood sugar and uric acid levels.
Nursing Considerations

>Assess pt's underlying condition before starting therapy >Monitor for renal, cardiac,
neurologic,GI manifestations of hypokalemia >Monitor for CNS,GI, cardiovascular,
integumentary, neurologic manifestations of hypocalcemia >Monitor for CNS,
hyperactive reflexes, depressed cardiac output, nausea and vomitting, Tachycardia
>Assess fluid volume status(urine, color, quality and specific gravity) >Assess pt's
tinnitus or pain.

Mechanism of Action

>Cofactor of many enzyme systems involved in neurochemical transmission and


muscular excitability; prevents or controls seizures by blocking neuromuscular
transmission; attracts the retains water in the intestinal lumen and distends bowel to
promote mass movements and relieve constipation.

Indications:
>IV or IM: preeclampsia or eclampsia. >To correct or prevent hypomagnesemia in pt's or
parenteral nutrition.
Contraindications
>Contraindicated with allergy to magnesium products; heart block, myocardial damage;
fecal impaction, intestinal and biliary tract obstruction >Do not give during 2hrs
preceding delivery because of risk of magnesium toxicity to the neonate
SE/AR
>SLE: syndrome joint-swelling, fever reaction
>SLE: syndrome joint-swelling, fever reaction, anemia, agranulocytosis, hepatitis,
glomerulonephritis, ARF.

Nursing Responsibilities
>Assess possibility for interactions with other drugs or herbal products. The patient may
be taking especially anything that may impact BP before start treatment >Monitor
patient's closely during and following infusions >Observe orthostatic precautions.

Responsibilities: Before:
• Check the doctors order
• Verify patients identity
• Asses GI output, bowel sound and liver evaluation
• Monitor blood studies
During:
• Administer on empty stomach 1 hr before or 2-3 hours after meals
• Do not administer with antacids
After:
• Assess for constipation or diarrhea
• Tell patient to verbalize stool, vomiting, rash, itching, vaginal sores
