AFFIDAVIT OF LOSS
That on early part of 2016, the said business has ceased its operations
due to some reason. While I was looking for the aforementioned official
receipts for me to be able to claim/ withdraw the bonds posted, I only found
a photocopy of the said receipts and despite of all the earnest and serious
efforts exerted to recover the original copies of the receipts, it proved futile
and still could not be found. Therefore, I considered it lost beyond my
recovery.

That on March 23, 2016, I found out that the said SSS ID, ATM and
senior citizen’s ID card were lost in my possession;
That despite of all the serious efforts that I have exerted to recover my
ID’s, the same could still not be found. Therefore, I considered it lost beyond
recovery.

That on November 11, 2018, I found out that the said driver’s license
card was lost in my possession, and despite of all the serious efforts that I
had exerted to recover the same, it proved futile and still could not be found.
Therefore, I considered it lost beyond recovery.

was lost in my possession, and despite of all the serious efforts that I had
exerted to recover the same, it still proved futile and still could not be found.
