AFFIDAVIT OF LOSS

I, ALVIN Z. YADAO, of legal age, married, Filipino and a resident of No.

842 Aranguren, Capas, Tarlac, after having been duly sworn to in accordance with
law, hereby depose and state: THAT-

1. I was issued a Company ID by Promo Edge Corporation;

2. I customarily kept said card inside my wallet;
3. Sometime in March 2017, I found out that said Company ID was missing;
4. I exerted all efforts to locate the same, but despite such efforts, I could no
longer find it. Hence, for all legal intents and purposes, the Company ID that
was issued to me is considered lost;
5. I therefore execute this affidavit to declare the fact of loss of the aforesaid
Company, and order to secure its replacement and for whatever legal
purpose it may serve.
