That, an Administrative Case against Cdt 3C John Mark Sumabat Nayre, for
Violation of Sec 3, Para A, sub Para 1 (a), Rule VI of the Cadet Guide, also known as
“Hazing, was filed before the Investigation Section of Cadet Affairs Section, PNPA,
Silang, Cavite.

That, I am the plaintiff in the above-stated case and I cause the preparation of
the foregoing complaint;

That, I have not filed nor commenced any other action or proceeding involving
the same act or omission with any other administrative disciplinary authority or forum, or
any other tribunal or agency;

That to the best of my knowledge, no such action or proceedings is pending

before any of said forum;

That, I have read the contents thereof and the facts stated therein are true and
correct of my personal knowledge and to the best of my knowledge and belief, and if
there I any action or proceeding which is either pending or may have been terminated, I
must state the status thereof;
