The undersigned Regional Prosecutor hereby charges the above-named respondent

with an administrative offense of Violation of Reasonable Office Rules and Regulation as
defined and penalized under Section 3, RULE V of 2017 Comprehensive BJMP
Administrative Disciplinary Machinery, committed as follows:

That on or about 16 November 2017, at Starling City Jail, and within the
jurisdiction of the disciplining authority, the above named respondent, designated
as Jail Warden, without regard to BJMP Rules and Regulations, did then and
(narrate acts constituting misconduct).

Acts contrary to BJMP rules and regulations.
