ACCOMPLISHMENT REPORT

In this October, year 2016 I continued as casual here at Caramoran Municipal Hospital,
an infirmary level; 10 beds capacity. I was reassign as Pharmacy Assistant in our Pharmacy, my
duties are assisting in dispensing, preparation the medication order, receiving and checking of
drug product, carrying out inventories, preparing charge slips, checking expiry drugs, making
papers bag for medicine and cleaning shelve and medicine, and I continue my duties at the Out
Patient Department are as follows, daily housekeeping, washing of instruments, daily
sterilization of instruments, specimen bottles & supplies as need arises, prepares cotton balls
weekly, gauzed, interview patient daily for consultation regarding their general data and chief
complaint; taking vital signs and records at the OPD chart; conduct health teaching daily such as
breastfeeding, personal hygiene, Newborn care, healthy lifestyle, hand washing, cough
etiquette, assist in retrieving patient record; fill up labs request forms, distributes patient
survey form, giving Ferrous Sulfate to OB patient, performing wound dressing, change linen at
the Out Patient Department, nebulization, and basic wound care. And also I help to prepare the
material for PHIC profiling.

These are my duties routinely done diligently.

ACCOMPLISHMENT REPORT

I was assigned as Casual Nursing Attendant here at Caramoran Municipal Hospital, for
the month of November, 2017, an infirmary facility with 10 beds capacity and was assigned as a
ward nurse. My duties and responsibilities are to assist the nurse on duty in admitting
patients, carry out doctor’s order such as witness consent, take baseline vital signs (blood
pressure, temperature, respiration in one full minute), advise diet appropriate to condition,
insert intravenous catheter and regulate its flow rate, perform bed making, assist or wheel
patient into his room assignment, orient to ward setting and policies. Moreover, I administer
medications as to its desired dosage, frequency, route and duration bearing in mind the 10
principles of right drug administration. I am also responsible in explaining and answering my
patient’s questions regarding the given medications, or specific procedures to be done like
suturing, internal examination, and any other invasive procedures, reinforce doctor’s
explanation and clarify what has the patient understood from what the doctor has told
him/her. Furthermore, I render nursing interventions specific to the patient’s case. For
instance, I render chest physiotherapy after nebulization, turn patient side to side every two
hours, maintain in a high back rest, encourage increase oral fluid intake for patients with
problems in respiratory system. For patients with cardiac problems, I make sure that I have
taken his/her vital signs hourly especially his cardiac and respiratory rate. For patients with
problems in gastrointestinal tract such as diarrhea which has blown up this October targeting
pediatrics, I make sure to check his hydration status, regulate fluid as to desired rate, offer oral
rehydration solution, collect stool and urine specimen, note frequency of vomiting and bowel
movement routinely, and impart health teaching especially about matters of disease
prevention and control.
In addition, I give health teachings to postpartum patients as to importance of
breastfeeding, proper positioning, good nutrition, perineal hygiene, immunization of her
newborn, proper cord care, appropriate diet and others. I also assist in suturing, remove and/or
insert foley catheter, dress wounds, and render newborn care.
These abovementioned activities are just a few of my duties and responsibilities done at
this institution

As I continue as casual nurse here at Caramoran Municipal Hospital, this November,


2015. I was assigned at the Out Patient Department. In the Out Patient Department my duties and
responsibilities are as follows; interview patient daily for consultation regarding their general
data and chef complaint, taking vital signs and records at the OPD chart. Data recording for new
and old patients, updating of OPD logbooks weekly, daily retrieving and filing of records and
master patient index, washing, sterilization of instruments and specimen bottles as need arises,
preparing pack of material such as sterile OS, cotton balls and specimen bottles. Performing
health teaching such as breastfeeding, Newborn Care, healthy lifestyle, personal hygiene’s,
proper hand washing, assist in removing of sutures, catheterization, minor suturing, and records
filing of Under Five Patients, provision of UFC Cards to mothers and Newborn babies, updating
weekly or monthly OPD reports, and also I help to assist the nurse on duty in admitting patients,
carry out doctor’s order such as take baseline vital signs (blood pressure, temperature, respiration
