BIOGRAPHICAL DATA
Biographical

The researcher was born on May 10, 1994 at Brgy. Tumiao, Lambayong,

Sultan Kudarat. She is the eldest among the two children of Mr. Joel L. Daquioag

and Mrs. Marilyn R. Daquioag who are presently residing at Brgy. Tumiao,

Lambayong Sultan Kudarat. She finished her elementary education at Tumiao

Elementary School, Sultan Kudarat and her secondary education at Notre Dame

Siena College of Tacurong.

Because of her desire to become a skilled nurse she decided to enroll at

Sultan Kudarat State University and she took up Bachelor of Science in Nursing.

She attended various seminars, trainings and workshop in and off campus

as enriched to her chosen profession.

First, I would like to proclaim the greatness of God, and that I wouldn’t

have made it without His guidance. Truly, He is the King of kings and Lord of

Lords.

My grandmother told me, that parents are next to God, and that is why,

they will be in second place of whom I would like to give my thanks.

To my Papang and Mamang who have exerted big effort to support me

financially and emotionally, to my caring sister AJ who are my best buddy and

my true best friend, I love you all and I really appreciated what are you doing for

me. You complete the spices of my success.

To my grandmother “ Mamang Baket” who has pushed me hard enough

for me to have the strength to prove that I am not a failure, I would like to give

her the assurance that she has taught me well to know when to walk away and

not to settle anything less than what I deserve.

To my cousins, Angeli, Van, Kaye, Runiel ,Hiedi, and Ruel whom I have

dragged with all my dramas in life, thank you for being there and for giving me
