AFFIDAVIT

I, MARY A. MANGADAP, of legal age, widow, Filipino, with

residence and postal address at Dulao, Lagawe, Ifugao after having
been duly sworn to in accordance with law depose and state that:

I am married, in accordance to Ifugao customs, to Wilson A.

Mangadap sometime in 2006 without the benefit of a legal
marriage;

I have a child out of my marriage with Wilson A. Mangadap;

That I am executing this Affidavit to attest to the truthfulness

of all the foregoing and for all legal intents and purposes it may
serve.
