Please be advised that the management have received reports concerning <VIOLATIONS>.

These incidents are in gross violation of the duties and responsibilities as written in your contract, specifically
the following:
 Compile, copy, sort, and file records of office activities, business transactions, and other activities;
 Maintain and update filing, inventory, mailing, and database systems, either manually or
 using a
computer; 


As you may be aware, these acts constitute sufficient ground for disciplinary action, including suspension and
termination from service. Considering the gravity of the offenses and that it is already the second time that
you have committed similar infractions, you are now required to submit a written explanation within five (5)
days. This is an opportunity for you to explain and defend your side. Failure to submit within the deadline
means you have waived your right to be heard and management may make a decision.
