AFFIDAVIT OF LOSS

I, JEAN GREY MONTOYA, of legal age, Filipino, and with office

address Unit 1303, The One Executive Offices, Barangay Nayong
Kanluran, West Avenue, Quezon City, Metro Manila Philippines, after
having been duly sworn to in accordance with law do hereby depose and
say, THAT:

1. I am one of the Billing Assistants of Paramount Human Resources

Multi-Purpose Cooperative (hereinafter referred to as “the
company”), a Cooperative duly organized and existing under the
laws of the Republic of the Philippines with office address at Unit
1303, The One Executive Offices, Barangay Nayong Kanluran,
West Avenue, Quezon City, Metro Manila Philippines;

2. One of my primary duties is to keep the original copies of the

Service Invoices of the company inside my office desk including
service invoice with control number 32821, 32822 and 3823;

3. Sometime in February of this year, I was looking for the original

copies of the Service Invoice with control numbers 32821 32822 and
32823 but to my surprise, said original copies of the numbered
service invoices could not be found;

4. I took pains to look for said original copies, but to no avail. In fact,
I look over records upon records in our office just to look for the
said original service invoices but still I failed to locate it.

5. As such, I am executing this Affidavit voluntarily and within my

own free will and to attest to the truth of the foregoing and
whatever legal purpose it may serve.
