Respondent, Donald Smith, hereby submits this plan of compliance in Consideration of the State agreeing to voluntarily dismiss the Petition for Civil Commitment pending in this cause, and agrees to obtain treatment as follows: 1. Respondent will immediately enter the River Region Drug Treatment Program (on an in-patient basis and remain there until in-patient treatment is completed. Respondent will continue to obtain necessary substance abuse treatment as directed by River Region until successful completion of the follow-up ‘outpatient program. River Region will provide monthly reports to Petitioner which details Respondent's progress in treatment. 2. Upon release from the in-patient portion of the River Region program, Respondent will reside with his mother, Patricia Moore, at 2495 Segovia Av., Jacksonville, Florida, 32217, phone 737-5215. He shall not change his address without first obtaining the permission of the Petitioner. tte ~ ~ 3. Respondent will receive outpatient psychosexual therapy through Dr. James Vallely, 8049 Arlington Expressway, Suite 12, Jacksonville, Florida, 32211 ‘The initial psychosexual evaluation is scheduled for Thursday, April 18, 2002, at 2:00 P.M. Thereafter, Respondent shall attend psychosexual treatment at the direction of his psychosexual therapist. The cost of such psychosexual evaluation and treatment shall be paid by Respondent. Respondent will attend Psychosexual treatment as directed by his psychosexual therapist until successful completion of such treatment. During Respondent's in-patient treatment at River Region, transportation to and from psychosexual treatment will be provided by Respondent's mother, Patricia Moore. Dr. Vallely shall submit monthly reports to Petitioner detailing Respondent's progress in treatment 4. Respondent shall ‘cceive I om Dr. Fawaz Ashouri and Continue to receive such shots until successful completion of psychosexual treatment or until such time as those shots are deemed to be no longer necessary by his psychosexual therapist, Said treatment shall begin no later that 30 days from the date of his initial evaluation by his psychosexual treatment provider. Failure to complete treatment as outlined above, or committing any offense against the criminal laws of the State of Florida may result in the State reinstating the Petition for Civil Commitment and confinement. 6.
