AFFIDAVIT OF LOSS

I, ELOISA D. AVILA, of legal age, Filipino, with residence and postal

address at 719 7th street San Martin subdivision Angono, Rizal, after having been
duly sworn to in accordance with law, hereby depose and state:

1. That I am a graduate of Angono National High School for the school

year 2002-2003.
2. That due to the fact that I changed my residence from Quezon City to
Angono, Rizal, some of my personal belongings got lost;
3. That due to an unforeseeable circumstance, my diploma was one of
those valuables that could not be located anymore;
4. That despite diligent search and efforts to locate the said High School
Diploma, I could not find it, such that I now believe that it is now lost
beyond recovery
5. That I am executing this affidavit to attest to the truth of the foregoing
and for whatever legal purpose this may serve.
