This is to inform you that you are one of the respondents of the School-Based Action Research
“IMPROVING MATHEMATICS ACHIEVEMENT AND POSITIVE ATTITUDES OF STRUGGLING STUDENTS
THROUGH GAME-BASED AND ACTIVITY-ORIENTED ASSESSMENT”. Kindly answer the following set of survey
questionnaire that measures your level of appreciation regarding your Mathematics subject. Rest assured that
