AFFIDAVIT OF SUPPORT

We, spouses RICHARD O. GOMEZ and LUCY T. GOMEZ, both of legal

age, Filipino citizens, and residing at 50-2 Damaso Suazo Street, Davao City,
after having been duly sworn to in accordance with law, do hereby depose and
state;

1. That we are the legitimate parents of JULIANA T. GOMEZ, who is 17

years of age;

2. That the above-named daughter will be traveling to South Korea on

February 14, 2019 for about a month-long vacation with our sister and
her aunt, IMELDA G. PAPIN, who resides at 434 Sunset Boulevard,
Miami, Florida;

3. That since our said daughter is travelling alone and is still a minor, we
hereby give our consent freely and voluntarily for her to travel to the
United States on said date;

AFFIDAVIT OF LOSS

I, PIOLO P. PASCUAL, of legal age, Filipino, single and a resident of 67

2nd Avenue, Alpha Executive Homes, Matina Aplaya, Davao City, after having
been duly sworn to in accordance with law, do hereby depose and state;

1. That I am a duly-licensed driver in accordance with pertinent Land

Transportation laws, rules and regulations, and was issued a
corresponding Professional Driver's License with number L02-09-
006768 which is valid until 13 August 2022;

2. That sometime on December 21, 2018, said driver's license was

misplaced and lost;

3. That efforts were exerted to locate said driver's license, but in spite
of diligent search, it could not be found and the same is now beyond
recovery;

4. That said Driver's License has not been confiscated by the LTO, Police
or other Traffic Enforcers for any traffic violation;

5. That as such, I am executing this Affidavit of Loss to attest to the truth

of the foregoing and to support my application for the issuance of a
new Professional Driver's License, in lieu of the one that was lost;
