St. Anthony College of Roxas City, Inc, or SACRI in compliance with Republic Act No. 10173 (R.A.10173),
otherwise known as the “DATA PRIVACY ACT OF 2012”, and inspired by one of our core values, “Respect for
Human Dignity”, would like to ask from you or your authorized representative, your free and voluntary
consent to collect, use, process, share, store and dispose your personal/sensitive information, with strict
confidentiality, as it deemed necessary, appropriate, vital and within the bounds of law, as you seek
educational services and/or do educational transactions with us.
SACRI collects and retains your personal/sensitive information such as your name, address, age, date of birth,
contact number, health records, program studied, assessment of payments, examination and result, by
physical and electronic data recording system upon enrolment/ admission and throughout your studies for
delivery of quality education services and/or during the course of your educational transactions with us,
within a reasonable time as allowed by law.
SACRI may use, share or disclose your personal/sensitive information for the following purposes:

The right to be informed whether personal data is being or has been processed; including processing
by automated means;
The right to require the school to correct any personal data relating to the data subject which is
incorrect or inaccurate;
The right of reasonable access to the personal data processed by the school;
The right to obtain a copy of the data subject’s personal information in the computer system of the
school;
The right to know the recipients of the data subject’s personal information processed by the school
and the reason for such disclosure;
The right to update;
The right to object;
The right to withdraw consent;
The right to erasure or blocking;
The right to damages;
The right to file a complaint;
Right to Portability and Transmissibility of Rights with limitations as stipulated in the Data Privacy Act
of 2012.
