CHAPTER ONE

INTRODUCTION

1.1.Background

The use of natural products to enhance wound healing is a common practice in many parts of

the world. The first written reference to honey, a Sumerian tablet writing, dating back to

2100-2000 BC, mentions honey's use as a drug and an ointment (Mandal,2011). A survey

conducted by Hermans1 in 1998 to review worldwide use of different treatment options for

burns found that honey was used in 5.5% of instances.

Honey was used as a topical cream for wounds in many African countries. It was used as a

component of beauty creams and for treating large wounds in Egypt (Subrahmanyam, 1999),

in somalia, honey is used as a topical .

Honey is a mixture of sugars prepared by honey bees from the natural sugar solutions - called

nectar - obtained from flowers or other plant secretions. By inverting the sucrose in the

nectar, the bee increases the attainable density of the final product, and thus raises the

efficiency of the process in terms of caloric density. By the addition of enzymes and the

evaporation of water contained in it, honey bees transform it into a sweet liquid . Scar is a

healing tissue usually called fibrosis.

1.2.Research Problem

Healing of a burn wound can occur either in regaining its epithelial tissue (regeneration)

or by formation of scar (williama et al, 2006). Regeneration gives the skin the ability to

gain its full function. Application of honey to the partial thickness burn speeds up the

healing process and prevents infection with shorter time of reepithiliazation .

There is no literature on the impact of the local application on the prevention of eschar

and scar formation

1.3.Purpose of the study

The purpose of this study is to determine and describe the impact of local application on

prevention of post-healing complications using clinical trial study design with the view of

improved prevention in burn patients in Madina Hospital.

Topical application of honey is charecterized by the application of honey on burn wounds

after cleaning and prevention of post-healing complicaitons by reduction of number of

eschar and scar formation and limitations of joint movement after healing of burn

wounds.

In the view of this operation, the study will determine the effect of topical application of

honey on the prevention of the formation of eschar, scar and limitation of joint movement

The specific objective of this study is to:

1. Determine the impact of topical honey on prevention of Escher formation

2. Establish the effect of topical application of honey on the prevention of scar

formation.

1.5.Research questions

The study will seek to answer the following questions:

1. What is the relationshsip between the topical honey and prevention of Escher

formation in burn wounds in Madina Hospital?

2. How does topical application of honey effect the prevention of scar formation

in burn wounds in Madina Hospital?

1.6.Significance of the study

This study will have a great impact on the uses of honey in wounds as a topical

application through its recommendations to the amounts to be used and the general

reduction of the number of scar formation and contractures in burn patients.
