JOINT-AFFIDAVIT OF TWO (2)

DISINTERESTED PERSONS

We, RODRIGO B. DERILON and ROMEO R.

BARRIENTOS, both of legal age, married, Filipino citizens
and residents of Brgy. Bintawan Sur, Villaverde, Nueva
Vizcaya, after having been duly sworn to in accordance with
law hereby depose and state:

1. That we know personally FRED REVILLA YAMBALLA

who is our longtime acquaintance and neighbors in
Barangay of Bintawan Sur, Villaverde, Nueva Vizcaya;

2. As such, we know that her true and correct name is

“FRED REVILLA YAMBALLA” and he has been using
the same in all his transactions, documents and other
pertinent records;

3. However when he secured his Certificate of Live Birth

from the National Statistics Office, now called Philippine
Statistics Authority (PSA), Bayombong, Nueva Vizcaya,
he discovered that the last name of his mother was
inadvertently recorded as “RIVELLA” instead of
“REVILLA” and his birth date was not readable but we
know for a fact that his true birth date is June 8, 1956;

4. He also secured his Certificate of Baptism in Saint Louis

Beltran Parish, in Solano, Nueva Vizcaya and he
discovered that his first name was inadvertently
recorded as “ALFREDO” instead of “FRED”;

5. We attest to the fact that “FRED REVILLA YAMBALLA”

and “ALFREDO REVILLA YAMBALLA” is the same as
FRED REVILLA YAMBALLA and his birth date is June
8, 1956;

6. That we attest to the veracity of the foregoing

statements;

7. That this affidavit is hereby executed for whatever legal

intent or purpose it may serve under the foregoing
premises.

IN WITNESS WHEREOF, we have hereunto affixed our

signatures this 6th of December 2018 at Bayombong, Nueva
