Affidavit of Denial

I, ___________________________ 36 years old, male, married, a resident of Sitio

Dahon, Cebu City, after having been duly sworn in accordance with law do hereby depose and
state;

That I am a bona fide member of the Philippine National Police since September 1, 2005,
presently assigned at Cebu City Police Station, Province o Cebu and further designated as
Warrant/Subpoena PNCO;

That on November 29, 2018 at around 7:00 p.m., I was informed by our radio operator
that there was a radio message coming from Provincial Investigation and Detective
Management Branch (PIDMB) directing me to report to the said office;

That at around 1:00 p.m. of November 30, 2018, I reported at the said office and one of
their personnel informed me that my namesake, with the rank of Senior Police Officer 1
(SPO1) appeared in the e-warrant database with a criminal case for Frustrated Homicide filed
on May 31, 2010 at the court of Lipa, Batangas;

That I hereby unequivocally deny involvement in the aforementioned case. I also

declare under oath that I am not in any way involved in any case proceeding as either
plaintiff/complainant or defendant/accused in the Philippines and abroad, especially with
regard to the aforementioned case;

That I am not the same person reflected on the e-warrant database for I have never
been to Lipa, Batangas and never been involved in any criminal case. Moreover, since I entered
the service all of my assignment as police officer are in region 7 and in CY 2010 my rank was
still a Police Officer 1 (PO1) as reflected in my PNP service record unlike the one in the database
which is SPO1 ;

That to substantiate my claim that I am not the same person on the said database, I have
a clearance from the National Bureau of Investigation (NBI) with remark that I have no
derogatory records since the aforementioned case was filed on the year 2010 until the validity
of my clearance on October 10, 2018, submitted as Annex “A”. For other references or
verification I will also submit my birth certificate, submitted as Annex “B”, PNP personal data
sheet and updated service record to prove that I am still active in the service, submitted as
annex “C”;

That I am executing this sworn affidavit to attest to the veracity of the foregoing facts
and to deny absolutely and entirely any involvement in the case aforementioned.
