AFFIDAVIT OF DISCREPANCY

That I, CASAMIRO CARLITO DELA CRUZ, of legal age, Filipino, married,

resident of and with a postal address at Poblacion H., Camiling, Tarlac, Philippines,
after having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am the registered owner of a parcel of land known as located at

Barrio of Poblacion H., Municipality of Camiling, Province of Tarlac with an
area of TWO HUNDRED (200) SQUARE METERS, more or less, covered By
TCT No. 431019;

2. That in the above-said subject parcel of land my name appearing and

mistakenly entered therein is “CARLITO CRUZ” instead of “CASAMIRO
CARLITO DELA CRUZ” which is my true and correct name;

3. Also, the Barrio appearing therein is “POBLACION 1ST” instead of

“POBLACION H.”, which they erroneously entered on the record;

4. That I hereby request the Hon. Register of Deeds of Tarlac to correct my

name from “CARLITO CRUZ” to “CASAMIRO CARLITO DELA CRUZ”
and and correct the barrio from “POBLACION 1ST” to “POBLACION H.”
after payment of the required legal fees, thereof;

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and facts.
