AFFIDAVIT OF CONSENT

I, ROSEMARIE T. SIA, of legal age, Filipino and a resident of Fortaleza St.,

Iligan City, after having been duly sworn to in accordance with law, hereby
depose and say that:
1. I am the owner/proprietor of a chicken house with business name
ANTON’S FRIED CHICKEN and business address at Fortaleza St., Iligan
City;

2. That the said chicken house is managed by my father BENITO R. SIA,

who is also a member of MSU-IIT Cooperative;

3. That my father is applying for a business loan with the said

cooperative;

4. That in view thereof, I am giving my consent to my father for the said

business loan before the MSU-IIT Cooperative since it was my name
who is registered in the business permit of the said chicken house;

5. That I am executing this affidavit to attest to the veracity of the

foregoing facts and for whatever legal purpose it may serve best.
