PROMISSORY NOTE

FOR VALUE RECEIVED, I, _________________________________________,

of legal age, single/married/widow, Filipino and a resident of
___________________________________________, promise to pay MARLENE
ESTEBAN, also of legal age, married, Filipino and a resident of Barangay Suizo, Tarlac
City, the sum amount of ____________________________________
(Php_________________), and interest at the monthly rate of ______________
(___%).

Repayment shall be made every two months consisting of _________________

per installment on the _________________________ day of each month until the entire
indebtedness is paid in full to be charged or debited against the borrower’s 4P’s ATM
Account maintained with the Land Bank of the Philippines (LBP) in my name under
Account No. _____________________. I voluntarily turned over the creditor the ATM
Card issued relative to the said account with the warranty/assurance that funds are
being regularly credited to the said ATM account which are more than sufficient to cover
the agreed upon monthly installment payments whenever due and demandable.

Borrower agrees that until the principal and interest owed under this promissory
note are paid in full, this note will be secured by her employer-issued ATM Card from
which the lender shall withdraw, as she is hereby authorized to do so by the borrower,
the payments due.

Borrower further agrees that he/she shall not execute any Affidavit of Loss
respecting the said ATM Card given as security without written permission from the
Lender. Neither shall the Borrower commit any act that would render the said ATM Card
is rendered invalid, borrower hereby authorizes the Lender to claim the payments due
directly from his/her employer - _____________________________ chargeable against
his/her salary.

In any event that collection on this note shall be made through legal action and
the lender prevails in the lawsuit, the borrower will pay the court costs, collection agency
costs and attorney’s fees in an amount the court finds to be reasonable.
