Guide

1. Employee fills out the form


2. All team leaders to conduct a group discussion
3. Team leader (Immediate Supervisor) assists the employee in fine tuning their
Goals and Objectives.
4. Team leader get together with their Functional Head (face to face or via
conference call) to review and edit the form. The objective of this review is
to make sure that the Goals and Objectives are in line with the Company’s
Goals and Objectives.
5. Team leader finalizes the form with the individual employee.
6. Both the employee and the team leader sign the form and send it to the
functional head for signature.
7. The functional head reviews each form with the CEO or a Vice President
designated by the CEO. Changes may be made as a result of this review.
8. The functional head finalizes the goals and objectives and gets it signed by
the team leader and the employee before providing it to HR.
9. A copy is to be retained by the Functional Head, Team Leader and the
Employee.
10. The immediate supervision conducts the monthly review of the objectives to
recommend remedial measures (if any).
11.The Functional Head to submit a report on each of their group members at
the end of the period with their recommendations.
12. An external (Other than the Functional Head) conducts an audit for final
recommendations.
