AFFIDAVIT

I, SAMUEL MUNYI GITHAE, of post office Box 9560-00300, Nairobi within republic of Kenya of Identification
Card 10888446 do hereby make Oaths and state as follows:-

1. THAT I am a male adult of sound mind, hence competent to swear this affidavit.

2. THAT I am the registered owner of a motor Vehicle registered No. KAW560D MITSUBISHI LORRY/
TRUCK.

3. THAT the number (Rear) plate of the above mentioned motor vehicle got lost.

4. THAT the matter was reported at the police station and an abstract was issued.

5. THAT I therefore Request the Commissioner and the Registrar of vehicles to issue another plate.

6. THAT I swear this affidavit conscientiously believing everything deposed herein is true to the best of
my knowledge and in accordance with Oaths and statutory declaration Act CAP 15 of the laws of
Kenya
