NOTICE TO ATTEND A DISCIPLINARY HEARING

You Ms. Tsakani Phakula as an employee of Ntsangalala Holdings Pty (Ltd) are hereby
notified to attend the disciplinary hearing on the charges levelled against you by the
Company. The disciplinary hearing is scheduled for the 30th October 2018, Tuesday at
11:30 at Head Office White River.

CHARGE ONE: FAILURE TO ACCOUNT FOR THE LOSS OF SITE DIESEL

You are charged with misconduct in that in your capacity as a General Labour on or about
the months of September 2018 to October 2018, you failed to account for the loss of +- 1994
litres of Diesel at Tembisa site. Your conduct is in violation of the Ntsangalala Holdings
disciplinary code and procedure.

CHARGE TWO: GROSS NEGLIGENCE

You are charged with misconduct in that in your capacity as a General Labour on or about
the months of September 2018 to October 2018, you were negligence about the keys of the
Diesel Tank at in that you neglected and or left the keys of the Diesel Tank unattended or
near Tembisa site. Your conduct is in violation of the Ntsangalala Holdings disciplinary code
and procedure.

CHARGE THREE: FAILURE TO REPORT

You are charged with misconduct in that in your capacity as a General Labour on or about
the months of September 2018 to October 2018, you failed to account for the loss of +- 1994
litres of Diesel and failed to report the loss of Diesel. Your conduct is in violation of the
Ntsangalala Holdings disciplinary code and procedure

CHARGE FOUR: GROSS DISHONESTY IN THE WORKPLACE

You are charged with misconduct in that in your capacity as a General Labour on or about
the months of September 2018 to October 2018, you failed to account for the loss of +- 1994
litres of Diesel and you were dishonest in disclosing the relevant information that may assist
employer. Your conduct is in violation of the Ntsangalala Holdings disciplinary code and
procedure.

CHARGE FIVE: FRAUD AND MISINTERPRETATION IN THE TIMESHEETS OF THE
EMPLOYER

You are charged with misconduct in that in your capacity as a General Labour on or about
the months of September 2018 to October 2018, you fraudulently changed the initial time
sheet and added overtime in a copy of the time sheet submitted to the employer. Your
conduct is in violation of the Ntsangalala Holdings disciplinary code and procedure

You are entitled to:

 Any representative provided that such representative is not a legal practitioner who is
admitted as an attorney.
 call any witnesses on your behalf;
 cross question any witnesses;
 an interpreter

PLEASE NOTE: the meeting can be held in your absence should you fail to attend.
