JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, DOMINGA CRUZ and EDUARDO LACAP, both of legal age,

Filipino citizens, and with residence and postal address at Brgy Matatalaib,
Tarlac City, after having been duly sworn to in accordance with law, do
hereby depose and say, to wit:

1. That we are acquaintances of RODRIGO M. TABAG who is the son of

the late CONSOLACION MERCADO;

2. That in the Certificate of Baptism of Consolacion, her name appearing

therein is “CONSALACION MERCADO” instead of CONSOLACION
MERCADO;

3. That in the Certificate of Live Birth and from the record of Social
Security System (SSS) of Consolacion, her name appearing therein is
CONSOLACION MERCADO which is her true and correct name;

4. That also, and from the record of Social Security System (SSS) of
Rodrigo Tabag the address appearing therein is Mapalacsiao Tarlac
San Sebastian instead of “Concepcion Tarlac”;

5. That “CONSALACION MERCADO” and “CONSOLACION MERCADO”

refer to one and the same person;

6. That the true and correct address of Rodrigo Mercado Tabag is

Concepcion Tarlac ;

7. That we are executing this affidavit not for any remuneration

whatsoever but to attest to the truth of the foregoing statements and
for whatever legal intents and purposes it may serve.
