AFFIDAVIT OF CONSENT TO TRAVEL

I, JOSEPH A. TOLLO, of legal age, Filipino, and a resident of San Vicente, Panabo City,
Davao del Norte, after having been sworn to an oath, hereby state that:

1. I am the biological father exercising parental authority over AJ RIZALDO TOLLO,

with Passport No. EC7783557, 15 years of age and a resident of San Vicente,
Panabo City, Davao del Norte;
2. Said child will travel to Malaysia to meet his mother, ANNALIZA FUENTES
RIZALDO, and to have a vacation;
3. Said child will be leaving on April 29, 2018;
4. Said child will be taken care of by his mother;
5. I am giving my full consent for the said minor child to travel to Malaysia;
6. Said child will be accompanied by his uncle, BENEDICTO JR. FUENTES RIZALDO,
with Passport No. P5811993A;
7. I execute this affidavit to attest to the truth of the foregoing facts and for whatever
legal purposes it may serve.
