1.0 INTRODUCTION
Policy on Health, Safety and Environment
Matrix is committed to conducting its operations in a safe and environmentally responsible
manner. To support this commitment, Matrix has a Policy on Occupational Health, Safety and
Environment. This policy is provided to those contractors working on behalf of Matrix and is
intended to be posted as a reminder of the company’s obligation to worker safety. A copy of the
policy is provided on Page 4 of this introduction.
In addition, Matrix has developed a Policy on Discipline and Enforcement. This policy is
included on Pages 6 and 7 of this introduction.
Participation in Industry Initiatives
Matrix is committed to “ensuring a strong future for our industry, the environment and the
people of Canada”.
 Matrix participates in the ‘Partnerships’ Certificate of Recognition program through Alberta
Human Resources and Employment and is a member of both the Alberta Association of
Safety Partnerships (AASP) and Petroleum Services Association of Canada (PSAC).
 Matrix endorses the Petroleum Industry Guiding Principles for Worker Safety included on
page 5 on this manual.
Occupational Health, Safety and Environmental (OHSE) Management System
Matrix Drilling Fluids Ltd. has developed an Occupational Health, Safety and Environmental
(OHSE) Management System based on the principles of ISO 14000. The foundation for this
process is described in the following diagram.

To fulfill this commitment, Matrix has developed a comprehensive Safety Program Manual. An
overview of Matrix’s program is highlighted on the cover of the manual. This overview outlines
the range of plans and programs that Matrix has developed to address the health, safety and
environmental issues resulting from our operations.
As part of our overall OHSE Management Strategy, goals and objectives are established annually
by Matrix management for key areas important to the Company’s performance. The company is
committed to recognizing and rewarding the contribution of employees and contractors in
achieving these goals. In addition, the company encourages its contractors to develop reward
and recognition programs appropriate for their operations.
Scope of Operations
Matrix designs drilling fluid programs and provides field support services to its Clients in one of
three ways:
1. Full Service Field Support: Matrix provides a Drilling Fluid Technician to be on-site
continuously to monitor drilling operations and ensure the effective implementation of
the drilling fluid program.
2. Partial Service Field Support: Matrix provides a local Drilling Fluid Technician to
periodically visit the wellsite to conduct a spot check to confirm drilling mud properties.
3. No Field Support: Matrix provides the Client with a copy of the drilling fluid program
and arranges for delivery of the required chemical additives. No field support is provided.
Safety and Loss Control Manual
This Safety and Loss Control Manual is designed to provide management, employees and
contractors with guidance and tools needed to address site safety management. The purpose and
scope of this Safety and Loss Control Manual and related documents are more clearly depicted in
the diagram on the following page.

Contractor Work Procedures


Matrix depends on both the Prime Contractor and its sub-contractors to develop detailed work
procedures specific to the activities they are responsible for. Specifically:
• Where required by safety regulations, contractors have developed and implemented any
required Codes of Practice or safe work plans (i.e. Confined Space Entry, Respiratory
Equipment, Fall Protection etc.)
• Contractors have taken the steps needed to ensure their employees and contractors are familiar
with the work and can complete any required work procedures.
• Operating and maintenance procedures are completed in accordance with manufacturer’s
recommendations.

The scope and content of this manual and the related procedures will be reviewed annually and
revised as Matrix’s operations expand.

The following are examples, but are not limited to, the actions which shall involve discipline
according to the aforementioned guidelines:

(a) Stealing or willful damage to property;


(b) Working while impaired by alcohol or drugs;
(c) Habitual tardiness;
(d) Smoking in areas where such is prohibited;
(e) Having open fires where such practice is forbidden;
(f) Carelessness in regard to accident prevention and/or safety of fellow
workers, and/or refusal to comply with Company, Client or Governmental Safety
Regulations;
(g) Failure to report or failure to report accurately to the proper authorities,
whenever required, accidents, operational breakdowns, personal injuries,
etc.;
(h) Insubordination, including refusals or failure to perform work at any time
when called upon to do so, unless work is considered excessive, unsafe or
illegal;
(i) Absence from duty without notice to, or permission from the immediate Supervisor;
(j) Sleeping while on duty;
(k) Obtaining materials from storehouse or other assigned places without proper authority;
(l) Malicious mischief resulting in personal injury or destruction of Company, Client or
worker property;
(m) Divulging confidential Company or Client information;
(n) Unauthorized use of a Company vehicle;
(o) Accumulation of traffic violations, which put driver’s license and/or Company vehicle
insurance at risk.

Notwithstanding the procedures outlined above, there may be circumstances where the nature of
the infraction is so serious that, upon review by Management, the employment of the offending
worker may be terminated immediately with cause.
