TITLE: Accident Notification & Investigation PROCEDURE: 001

Definitions (in relation to this Procedure)

Accident / Incident any undesired circumstance which gives rise to injury or illness, damage to
property, plant, products or the environment.

Near Miss an unplanned or uncontrolled event that in certain circumstances, could have
caused injury to people or damage to property, plant, products or the
environment.

First Aid emergency treatment administered to an injured or ill person before


professional medical care can be administered.

LWdC Lost Workday Case. An injury or illness that causes a person to be unable to
report to work for his next scheduled shift.

MajPD Major Property Damage. Any occurrence that causes damage to equipment
or plant, but no injury or illness to a person and the extent of the damage
exceeds US$5000 (SAR 18500).

MinPD Minor Property Damage. Any occurrence that causes damage to equipment
or plant, but no injury or illness to a person and the extent of the damage
does not exceed US$5000 (SAR 18500).

MajE Major Environmental Incident. An event causing environmental harm that


requires the assistance of external organizations, or a breach of Royal
Commission Environmental Legislation, standards or codes.

MinE Minor Environmental Incident. A badly controlled emission to land,


atmosphere or aquatic resource that has the potential for environmental
harm, or a substantiated complaint by residents or general public, regarding
the possible environmental impact from construction activities.

Procedure

1. Contractor is to immediately report all accidents involving fatalities, injuries requiring


hospital treatment and road traffic accidents to Royal Commission Security Department
on 03 341 7777.

2. Contractor is to verbally report to Royal Commission Construction (ES&H) Department
within 20 minutes of the occurrence of all fatalities, (suspected) LWdC, MajPD and MajE
Category of incidents.

3. Contractor is to verbally report to Royal Commission Construction (ES&H) Department


within 1 hour of the occurrence of all MinPD and MinE Category of incidents.

4. Contractor is to verbally report to Royal Commission Construction (ES&H) Department


within 12 hours of the occurrence of all First Aid and Near Miss Category of incidents.

5. Contractor shall submit written Accident / Incident Notification Form (an example is at
Appendix 1) within 24 hours to the Royal Commission Construction (ES&H) Department
for all incidents.

6. Contractor shall ensure that the scene of an accident involving a fatality or LWdC should
remain unchanged (unless there is imminent danger in not doing so) until Royal
Commission approval is granted.

7. Contractor shall formally investigate all LWdC, MajPD and MajE category of incidents,
identify root causes, and submit a full investigation report to Royal Commission
Construction (ES&H) Department within 72 hours.

8. Contractor shall implement all recommendations, corrective and preventative actions


identified during accident and incident investigations.

9. All incidents requiring First Aid shall be entered into a First Aid treatment register as
required by Standard 300 – Medical Services & Management. This register is to be
made available for inspection to Royal Commission at anytime.

10. Contractor shall report on a monthly basis all accidents and incidents as required by
Special Condition 21.
