AFFIDAVIT OF CONSENT

We, LILANIE ESPADILIA BALAURO, RUBE ESPADILLA, RIZA ESPADILLA

DELGADO, GIANELLE ESPADILLA, all of legal age, Filipinos, and residents of
Banilad, Dumaguete City, Negros Oriental, Philippines, having been duly sworn in
accordance with law, hereby depose and state that:

1. We are the co-owners of a portion land, designated as Lot. 4376-E located at

Banilad, Dumaguete City, having inherited the same from our late parents,
Claudia Avenido married to Benito Espadilla;

2. We have acquired the said property by virtue of succession;

3. We have granted permission unto Rex K. Balauro, to allow electric connection

on his house constructed in the portion of our share on the above mentioned
lot;

4. We hereby signify my written consent, freely, willingly and voluntarily to allow

Rex K. Balauro to apply for Electric service connection with NORECO II for his
house and his family located on aforementioned property;

5. We will take all responsibilities and obligations arising from the grant of this
consent to Rex K. Balauro. Hence, we undertake to hold free and harmless the
officers and employees of the City Government of Dumaguete and NORECO II
from any adverse claim or any conflict arising between the owner(s) and/or heir(s)
of the property involving herein service connection;

6. We are executing this affidavit to attest the truth of the foregoing facts as to our
undertaking to give consent to Rex K. Balauro to support his application for
electric connection.
