Application:

General Requirements
1. Duly accomplished and signed Australia Visa Application Form 1419 and Form 956
2. 2 pcs. ID Picture with white background (2x2 / Passport size)
3. Original Passport and copy (at least 6 Months Valid)
4. Original Old passport if you have visa of the following (used within the last 5
years):
Australia, Canada, Chile, Japan, Korea, Ireland, Israel, Mexico, New Zealand, Turkey,
United Kingdom, US, Schengen
5. Original Bank Certificate
6. Photocopy of Passbook
7. Photocopy of Time Deposit (If any)
8. Photocopy of Last 3 Months Bank Statement
9. Photocopy of Last 3 Months Credit Card Statement
10 Photocopy of Land Title Certificate
11. Photocopy of Car Registration OR / PR
12. Photocopy of any property documents (if any)
13. Photocopy of NSO Marriage Certificate (If traveling with spouse)
14. Photocopy of NSO Birth Certificate (If traveling with children)
15. Photocopy of PRC ID for professionals. (Doctors, Engineers, Accountants etc.)
16. Photocopy of IBP ID for (Lawyers)
17. Photocopy of Alien Certificate Registration/ACR (For Foreign Passport)
18. Photocopy of Immigration Certificate of Residence/ICR (For Foreign Passport)
19. Letter of support and sponsorship from UQ

For Student/Child:
1. Original School Certificate
2. Photocopy of School ID
3. Photocopy of Registration form
4. Confirmation letter (inconsistency in the picture photo)

If you are staying in Australia for more than three months, you are required to provide evidence of
adequate health insurance for the duration of your stay in Australia, such as:
o a certification letter from your health insurer or broker
o evidence that you have enrolled with Medicare if your country has a reciprocal health
arrangement with Australia
o the (139KB RTF file) completed by your insurance provider.

Evidence of your financial capacity to support yourself and your family while you are in Australia,
such as:

 bank statements
 a letter from your financial institution stating your financial position
 employment contracts
 documents showing allowances, accommodation and stipends from your sponsor while you are
in Australia.

 Evidence that the sponsorship application has been lodged or approved, such as:
o sponsorship application ID number or sponsorship Transaction Reference Number (TRN)
o sponsor approval letter.
 A written statement from the sponsor that they will meet the sponsorship obligations for all your
dependent family members you include in your application.

Applicants may submit their completed Visa Application form along with all the required supporting
documents in-person or by a representative via the AVAC Monday to Friday 7:00 - 14:00 (except public
holidays).
