AFFIDAVIT OF CANCELLATION OF ADVERSE CLAIM

I, JEMUEL A. LAZO, of legal age, Filipino Citizen, married, with residence

address at Brgy. Tiniguiban, Puerto Princesa City and in my capacity as President and
CEO of Concept Wealth Technology Enterprise, Inc., after being duly sworn to in
accordance with law, depose and say, that:

1. Concept Wealth Technology Enterprise, Inc., of which I represent, is the

adverse claimant in a Notice of Adverse Claim annotated at the back of Original
Certificate of Title (OCT) No. E-21953 registered in the name of LYDIA A.
ROXAS and issued by the Register of Deeds of the Province of Palawan. The land
covered by OCT No. E-21953 consists of FOURTEEN THOUSAND FOUR HUNDRED
THIRTY (14,430) SQUARE METERS, more or less;

2. The Notice of Adverse Claim was annotated as Entry No.

2013000559 dated March 27, 2013 at the back of OCT No. E-21953 which
reads as follows:

“NOTICE OF ADVERSE CLAIM: EXECUTED BY CONCEPT WEALTH

TECHNOLOGY ENTERPRISE, INC., REP. BY JEMUEL A. LAZO,
CLAIMING AMONG OTHERS THAT THE 1,800 SQ.MTS. PORTION OF
THIS PROPERTY IS A SUBJUECT OF THE DEED OF ABSOLUTE SALE
AND THAT DUPLICATE COPY OF TITLES IS STILL IN THE
POSSESSION OF THE VENDOR-LYDIA A. ROXAS, IN ACCORDANCE
WITH DOC. NO. 1060; PAGE NO. 12; BOOK NO. 5; SERIES OF 2013,
OF NOTARY PUBLIC OF PUERTO PRINCESA CITY, ATTY. RUSTICO P.
CACAL, DATED MARCH 26, 2013.”

3. I hereby request the Register of Deeds of the Province of Palawan to

cancel or cause the cancellation of the annotation of the Affidavit of Adverse Claim
or Notice of Adverse Claim annotated on OCT NO. E-21953

4. I am executing this affidavit to attest to the truth and veracity of all the
foregoing and for all other legal intents and purposes.
