AFFIDAVIT

I, PCINSP LORD WILSON JACINTO ADORIO, legal age, Filipino

and active member of Philippine National Police at Ramon, Isabela as
Acting Chief of Police/Field Training Supervisor after having been duly
sworn to in accordance with law, do hereby depose and state:

1. That on 13 April,2018 at around 6:30 o’clock in the evening,

Police trainee Judy Christ P. Apostol referred his problem to SPO2
RONALD I. FARNACIO and PO3 JESSIE R. BUNAO, the duty Field
Training Officers(FTO) of the police trainees in our station, regarding the
illness of his one(1) year old daughter named Johannah Reign C. Apostol;

2. That the said daughter of Police trainee Judy Christ P. Apostol

was then brought for check-up at SALAS DAUIGOY Family Medicine
Clinic at Barangay San Pedro, Tumauini, Isabela on 5 April, 2018 by his
wife;

3. That the wife of Police trainee Judy Christ P. Apostol called

him, informing that their daughter is still not feeling well and needs to be
brought to the hospital;

4. That Police trainee Judy Christ P. Apostol asked permission to

both SPO2 RONALD I. FARNACIO and PO3 JESSIE R. BUNAO to bring
his daughter to the hospital and as the duty Field Training Officers(FTO)
then;

5. And immediately thereafter, both SPO2 RONALD I.

FARNACIO and PO3 JESSIE R. BUNAO called my attention regarding
the situation conferred by Police trainee Judy Christ P. Apostol;
 6. That few minutes thereafter, I received a cellular phone call
from the wife of Police trainee Judy Christ P. Apostol informing the same
matter and pleading for humanitarian consideration since it is an
emergency situation;

7. That although it was already night time and after careful

evaluation of the facts and circumstances, for humanitarian consideration
and with consent from the Field Training Officers(FTO) on duty, I allowed
an emergency pass to Police trainee Judy Christ P. Apostol with strict
instruction to the latter not to ride with his own Yamaha Sniper 150
motorcycle vehicle but instead in a passenger vehicle bound to the said
place;

8. That I executed this affidavit to attest into the truth of the

foregoing and I carefully read and understood this affidavit and
freely and voluntarily affix my signature herein fully aware of its
legal implication;

IN WITNESS WHEREOF we have hereunto set our hands this 3rd

day of Mayl, 2018 at Santiago City.
